Add Responses

Required Permissions — Allow QA Form Administration

This task can be performed as part of creating the initial form, to edit an unused form at a later time, or to add a response to a new version of a form. If you are adding a response during initial form creation, begin with step 4.

  1. Follow the procedure for View QA Evaluation Forms.
  2. Locate the form in the list and click Edit.
  3. Locate the appropriate section, or click Create New Section and type a unique Section Title (50 characters max).
  4. Locate the question or follow the procedure to Add Questions.
  5. Click Add New Response.

  6. In the Response field, type the text of the response as it will appear to the evaluator.
  7. Optional: Select an Auto-Fail value.
  8. Select Default if you want inContact WFO to pre-populate the question with this response.
  9. In the Possible Points field, type the maximum point value for this response.
  10. Optional: Repeat steps 5-9 to continue adding responses to this question.
  11. Optional: Reorder the responses.
  12. Return to step 8 in the Add Questions task or click Save.
